Since they could not get him to Jesus because of the crowd, they made an opening in the roof above Jesus by digging through it and then lowered the mat the man was lying on.
King James Bible
And when they could not come nigh unto him for the press, they uncovered the roof where he was: and when they had broken it up, they let down the bed wherein the sick of the palsy lay.

Mark 2:4 Parallel
CommentaryClarke's Commentary on the Bible
They uncovered the roof - The houses in the east are generally made flat-roofed, that the inhabitants may have the benefit of taking the air on them; they are also furnished with battlements round about, Deuteronomy 22:8; Judges 16:27; and 2 Samuel 11:2, to prevent persons from falling off; and have a trap door by which they descend into the house. This door, it appears, was too narrow to let down the sick man and his couch; so they uncovered the roof, removed a part of the tiles; and having broken it up, taken away the laths or timber, to which the tiles had been attached, they then had room to let down the afflicted man. See Luke 5:19, and on Matthew 10:27 (note); Matthew 24:17 (note).

Thy sins be forgiven thee.--MARK 2:5. I will forgive their iniquity, and I will remember their sin no more.--Who can forgive sins but God only? I, even I, am he that blotteth out thy transgressions for mine own sake, and will not remember thy sins.--Blessed is he whose transgression is forgiven, whose sin is covered. Blessed is the man unto whom the Lord imputeth not iniquity.--Who is a God like unto thee, that pardoneth iniquity? God for Christ's sake hath forgiven you.--The blood of Jesus Christ …

News about him spread all over Syria, and people brought to him all who were ill with various diseases, those suffering severe pain, the demon-possessed, those having seizures, and the paralyzed; and he healed them.
When they could not find a way to do this because of the crowd, they went up on the roof and lowered him on his mat through the tiles into the middle of the crowd, right in front of Jesus.
